This notice explains how your medical information may be used and disclosed and how you can access this information. Please review this notice carefully.
Therapy and Medication Management Appointments:
It is very important that you complete the web registration before you schedule an appointment. Please note that some providers may reschedule your appointment if they find that the web registration is not fully completed.
Please save your login information and confirmation number in case your web registration encounters an unexpected error. Then, we can help you reload it.

Appointment Availability:
We are currently accepting new patients and will make every effort to schedule you for the earliest available appointment. Please note that for some of our more in-demand providers and services, you may be placed on a waiting list.
Our Policies:
Your privacy is important to us. Please understand that due to HIPAA regulations, we can only speak with the patient, the patient’s parent/legal guardian, or the designated person as assigned on your Consent form.
Insurance:
We are in-network with many commercial and government insurances, though not all. Each provider is credentialed with different insurances, so your specific policy will determine which provider you can see. Deductibles, co-pays, or co-insurances may still apply. Employee Assistance Programs (EAP) can only be used for therapy appointments. For details on therapists accepting EAPs and the specific programs we accept, please contact our office directly.

Medicaid/Anthem HIP:
We are not currently in-network with Medicaid (including Anthem HIP). However, if you have Medicaid, you are welcome to be seen as a self-pay patient. We'll keep a copy of your Medicaid card on file for prescription purposes. Please note that we won't submit a claim to Medicaid for your office visit, so you'll be responsible for covering the cost of your visit directly.
Ready to Schedule? What To Know Before Completing the Web Registration
If you are ready to schedule an appointment or testing services, please click the New Patient Web Registration button below and complete the Online Patient Web Registration and History sections. This process may take 20 to 45 minutes, depending on your medical and mental health history. You’ll have the option to save a password and return to finish it later. After submitting your registration, our office will contact you within 1-3 business days to schedule your appointment date and time.
New Patient Appointment Co-pay: $65
(Please note that a $65 refundable co-pay is required for all New patient appointments. This fee will be applied to your balance after your appointment or refunded.)
All New and Returning Patients: Please read the following carefully before you complete the secure, HIPAA-compliant online Web Registration and Medical History.

Appointment Policy
We look forward to meeting you at your scheduled appointment time. Please note that failing to provide prior notice for a missed appointment may prevent another patient in need from being seen that day. Another convenient option is to switch to a Telehealth appointment if you are unable to attend an in-person visit.
If you need to cancel an appointment, please give us at least 24 business hours' notice to avoid a charge.
Late Cancellation Fee: $65
(less than 24 business hours from your appointment time. (Your insurance will not cover this fee.)
No Show Fee: $90
(Your insurance will not cover this fee.)
Unless there are emergency circumstances as determined by you and your provider, the fees above will be charged for all late cancellations and no-shows.
Repeated late cancellations or no-shows may prevent us from continuing to provide care and may require us to release you from our services.
